Trophy Bassmasters Membership Application and Liability Release (REV 1-18-26)

RELEASE OF LIABILITY- READ BEFORE SUBMITTING THIS APPLICATION!

By submitting this information, the sender acknowledges, appreciates and agrees that all club sponsored events included but are not limited to tournaments, carry the significant risk of injury for the activities involved, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce the risk, the risk of serious injury does exist and YOU KNOWINGLY AND FREELY ASSUME ALL SUCH RISK, BOTH KNOWN AND UNKNOWN, EVEN IF ARISING FROM THE NEGLIGENCE OF TROPHY BASSMASTERS, OR OTHER PARTICIPANTS, AND ASSUME FULL RESPONSIBILITY FOR YOUR PARTICIPATION.

You and on behalf of your heirs, assigns, personal representatives, and next of kin agree to Hereby Release, Indemnify, and Hold Harmless the Trophy Bassmasters, and of its members, representatives, officers, or board of directors, sponsoring agents, sponsors, advertisers, or other participants, with respect to ANY AND ALL INJURY, DISABILITY, DEATH, OR LOSS OR DAMAGE TO PERSONAL PROPERTY, included but not limited to court cost and attorney fees, suffered or incurred while attending any club sponsored function.

You also agree to abide by the rules and by-laws set forth by Trophy Bassmasters. If, however, you observe any unusual significant hazard during your presence or participation you will remove yourself from participation and bring such to the attention of the Trophy Bassmasters official immediately. In agreement to these terms, you accept membership for the calendar year in which you signed the document. The terms of this application will remain in effect as long as the member is a member of the Trophy Bassmasters.

APPLICATION FEE OF $50.00 FOR THE CURRENT YEAR IS DUE UPON APPROVAL OF THIS APPLICATION.

*** Our club requires 300k of boating liability insurance for all boats used in the tournaments, Proof of insurance must be presented on request. A valid fishing license is required for all club fishing events.
